[A11Y-102] Scoped effect manager and cleanup lifecycle #33

Implement idempotent apply/dispose with scoped CSS/listeners/observers/overlays, dynamic-content limits, widget exclusion, portals and StrictMode safety. Never broad-rewrite React DOM or resume effects owned by another controller.

Acceptance: Enable-update-disable-unmount-remount leaves no stale effects or compounding.
