Replace the linear if-chain in set_fctx_from_dict() with a table-driven approach using offset tables generated from the new !generate-dict-table struct annotation. Move opening braces to their own line for C function definitions, consistent with the Linux kernel coding style used throughout libnvme. Also fix pointer spacing in PyDict_SetItemStringDecRef() signature (PyObject * p -> PyObject *p).
