Add field for date of birth

Adds an optional field for date of birth. Solves #49.

I prefer \textborn over the font awesome asterisk, but stuck to the general policy. Easily changed with

\usepackage{textcomp}
\renewcommand*{\bornsymbol}{\textborn~}

if one so desires.
