When Globus Auth is unavailable, users are unable to authenticate. Add username/password auth as fallback using a simple login form and view. NOTE: An admin must enable password-based login by setting the user's password via the admin site. The password login page is also not linked anywhere on the site, so users must know the URL to access it for now.
