feat: add Full Access and simplify permission tiers - #235

Introduce Ask, Approve for Me, and Full Access with session-only or remembered risk acknowledgment. Reset legacy permission settings to Ask and enable bounded model-error continuation in both automatic tiers. Preserve explicit tool overrides and scope consent to the active session. Route /exit through the existing /quit shutdown path, remove duplicate extension registration, and cover permission migration, consent lifecycle, and exit behavior.
