G4 migration: Zorg bij jou Thuismonitoring #4600

Current state
The only public definitions found (dev_homemonitoring2024.json, dev_homemonitoring_ura2024.json in nuts-foundation/discovery-service-definitions) require a NutsOrganizationCredential / URACredential, not an X509Credential, so they contain no did:x509 ca-fingerprint. To be confirmed with the use-case owner whether production definitions or access policies pin the UZI CA anywhere, and whether UZI server certificates are used for TLS between participants.
