Codex Pro Max SQL-Based Version

Release the SQL-backed Codex Pro Max Next system as the main project. The previous file-based implementation is preserved under old/. Includes setup.cmd, start.cmd, uninstall.cmd, backend session endpoints, SQLite persistence, rollout-based usage/thinking updates, and the migrated UI.
