Recover from failure to create Ibex Account #90
Description
Upon deploying to production, we were unable to create Ibex accounts. From this issue, we discovered that Ibex createAccount failures put our backend in a partial state that it is unable to heal from. A user who has this problem will continue to see failures.
Purpose of this task is to detect when a failure has occurred and re-initialize the Ibex account.
Testing:
- - Create integration tests (mock Ibex failure) to verify account is created successfully on second attempt
- - Verify happy path account creation still works in tests env

Priority pass 2026-10-10: rank 5 of 13. Partially fixed: #493 self-heals a missing USDT wallet on first touch. Still open: create-account.ts persists the account before wallet creation and returns an error on other failures, so an IBEX outage during signup leaves wallet-less accounts. Needs the integration test in the original checklist.

Handle: nul-quill. Payout wallets: Solana USDC BdznDjc4xwt5Y6WJT5UnrFQzDptka8YnC9oqmmkKpwkd, TRC20 TNEHCVwR2aPMrj5AKfWCQ2rFL3bEDQmsg7. Shipped in #522: ensureCashWallets() gap-fills missing cash wallets and repairs defaultWalletId, so an account left half-built by an IBEX createAccount failure heals on the next read instead of failing forever. Mocked IBEX failure plus second-attempt success covered in test/flash/unit/app/accounts/ensure-account-wallets.spec.ts; happy-path creation still asserted by create-account.spec.ts. 118 suites / 1431 tests pass.
