Add scoped ai: locators, fingerprinted cache replay/update/only modes, secret-safe cache entries, and one retry with a fresh plan when an ACT target fails.

Recovers the intended changes from #37, which were merged into the old development stack but did not reach main. Preserves the newer lithos-llm pin from #54.

Stage 3 of 10. Based on #58. Full recovery inventory and merge order: #57.

Merge the recovery PRs in order with merge commits. Once the preceding PR lands, retarget this PR to main.

Validation: CARGO_NET_OFFLINE=true mise run check passed on this branch. This runs formatting, Clippy, local Rust/browser tests, shim checks, eval tooling, and workflow checks. Paid model evals, the external BrowserSim round trip, and live Kernel checks were not rerun.

ai:"description" is a locator segment that a language model resolves to
one element. <flow>.whirl-cache.json records what each ai: target and ACT
line resolved to as strict generated locators with a role-and-name
fingerprint; runs replay it and heal a miss with a warning. --cache
selects replay, update, or only. An ACT action that fails heals once with
a new plan. The shim moves to protocol 6 with generateLocator.
