feat(opaque-links): resolve o- rooms through their ddocId (TEC-3037) - #33

- Store the on-chain ddocId on DocumentMeta when an o- room's owner sends it (write-once, pinned portal only); index ddocId + portal. - Publish reconciler checks the chain with ddocId ?? documentId and skips o- rows without one; newest rows first. - Deleted-file webhook matches _id or ddocId, portal-matched. - list-my-documents returns ddocId per row and deletedDocuments. Additive: rooms keyed by a ddocId and clients that send no ddocId behave as before.
