Prepare v0.4.2 with Code Review hidden by default - #492

Conversation
Val chose v0.4.2 with Code Review hidden until its live and human gates pass. Built apps now default the setting off, development builds default it on, and the old switch's value is honoured until the layout saves a choice. Bump the desktop version, start the release review, and correct the docs that described the setting as on by default. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

Prepares desktop v0.4.2, a patch release. Val chose this on 2026-11-03: Code Review stays hidden until its gates clear.
Changes
DEFAULT_LAYOUT.codeReviewis nowfalse, so a built app doesn't show Code Review and makes no Code Review read. Development builds still show it.modex.codeReviewswitch keeps its meaning.docs/status.md, README, the design doc, the CR-8 matrix and Val's session doc now describe a hidden default. The v0.5.0 candidate doc is marked superseded.Tests
A new e2e test covers the hidden default: the rail leaves Code Review out, no
ghread runs, and Settings can show it. The other Code Review specs seed the switch again.Local run on
