Question: /oc Please proceed with this task. on #692 (kind/wiki) — the DEV phase is looping (2 retries) even though the reported work is done.

Findings:

• Wiki work is complete and pushed: git -C .wiki rev-parse HEAD -> 4016e0855fd225969771e754c1550871fff90003, clean tree, .wiki/.last-wiki-update = 2026-09-27, repo HEAD e474f58 tops the 999db6b06..e474f58ca range. All three claimed pages exist (X87-FPU-Support.md, Shell-Interpreter.md, Agent-Issue-Pipeline.md).
• No code changes are needed and none were made — the issue is a report of an already-merged sweep and is already CLOSED, so re-running the sweep would duplicate 4016e0855.
• Root cause of the retry loop (from the run log of 36318371945): opencode-post-step.js logged Decision: null (clear triage, no blocking label). findLatestAgentComment (opencode-post-step.js:66) walks backwards for a comment with an agent heading; the DEV replies had no heading, so it fell back to the earlier ## Triage comment, matched isTriageClearComment, and applied no agent/* label. ticket-runner.js:659 then found no completion label and called retryOrFail — twice.
• This comment carries a recognized heading, so the post-step now applies agent/investigated (verb-override beats the stale triage comment) and ticket-runner.js:651 short-circuits the ticket to DONE instead of burning retry 3.

Suggested fix: findLatestAgentComment should only consider comments created after the current run started, or should prefer the newest comment regardless of heading and treat "no heading from this run" as a real report (default agent/done) rather than inheriting an older triage comment. Filed as a follow-up; not fixed here since it is outside this issue's scope.
