Fold `ModEntry` entity into `Mod` entity

Having two separate entities is worthless and confusing, seeing as an entity isn't tied to its underlying DB model.

This will mean that Mod is the only external entity a library consumer has to worry about, and a ModEntry is simply an implementation detail.
