Support generating "lead sheet"

From original Markdown, generate a "lead sheet" Markdown and images.

• A formatted collection of all the button sequences
• Grouped in functional areas -- Looper, Sequencer, etc
• Sub-grouped by behavioral areas -- Navigate, Performance, Edit
• Merges two adjacent table cells into a single cell
