No named owner for AI-context/skill lifecycle: automation health, human review, and stale-context cleanup are ad hoc. lscache refresh and skills exist but nobody owns keeping the context layer fresh.

Names the context-refresh DRI (Phil Henning; backup/reviewer Emanuel Fernández Dell'Oca), delegates per-doc accuracy to the existing docs/v2/** SMEs, and folds the drift-review cadence into normal milestone/sprint planning. Satisfies both acceptance criteria.
