Replace text affiliations with institutional logos (UC Davis / ASU / Morgan Stanley)

Logos sit in an equal-width row (object-contain in fixed-size cells), each
linking to the institution's homepage. Author dagger/double-dagger marks
removed since they no longer reference footnotes.
