Sound volume drops after a phone call

After receiving a phone call, all sound volume drops to circa 40% of previous sound level.
Volume control shows no change, but all sounds (ringtone, in-call, in-call-speaker, media playback, alarm) are at 40% of previous sound level.

Rebooting the phone fixes it, but only until the next call is received.

Steps to reproduce, on a freshly booted device:

1. Start YouTube video
2. Receive phone call, answer it.
3. Continue playing YouTube video
