Subject: [PATCH] docs: add constants module to README, AGENTS, CHANGELOG
 (BL-018 Feature 003)

- Update README.md Modules section with constants entry
- Update AGENTS.md Project Structure diagram with constants.py and test_constants.py
- Add v004 entry to docs/CHANGELOG.md documenting constants module addition
