Support out-of-order streaming (the v2 noted in the README)

Today deferred parts stream in document order. Out-of-order streaming (slot content as each resolves, regardless of order) needs a tiny inline placeholder script. The README calls this the natural v2. Design + implement behind an opt-in flag.
