MaxVolumeFragmentsPerPixel is too low at 16 for this game. 32 is still not enough.
I may have to revert to using a linked list for volume fragments instead of the current fixed size array.

Edit: It's still not perfect at MaxVolumeFragments=64 and MaxVolume=32 and I'm afraid of the performance impact. I'll consider increasing these limits a bit (MaxVolumeFragments=32 for example) if other games exhibit the issue, but I don't think it's worth fixing for now.
