Shigellosis - Enhance case data form

Problem Description

After the Disease enum has been modified in (#13927) as part of epic (#13926), the case data form needs to be updated with Shigellosis -specific details

Added Value/Benefit

SORMAS will support Shigellosis disease case surveillance

Proposed Solution

• Info button needs to display.
• In Pre-existing condition section, hide the Asplenia & Down syndrome.
• Introduce the following new conditions with YesNoUnknown
  • Chronic disease, if YES, display the text field.
  • Under medication(other than the current disease), if YES, display the text field.
• Other conditions should display as it is.
• Vaccination status field should be hidden, while the disease don't have any recommended vaccine
• Case person should function as other LUX diseases.
  • Living status and birth country is not LUX, Entry date should display.
