Add Support for signing a CSR with a certificate from a different Key Vault instance

For the tools AzureCreateIntermediateCert, AzureCreateSigningCert, AzureCreateSslCert, add support to

• sign with a certificate located in a different KeyVault instance
• Access to the second Key Vault should be done using one identity for both vault instances
