• Data Structure Description - free text. Required.
• DataCaptureStartDate - date. Required.
• DataCaptureEndDate - date. Required.

If you want to test right now, I can build a new portal in about 12 minutes. Otherwise it'll rebuild overnight and you can test the restored form tomorrow (April 18)
